DESCRIPTION:The Care Navigation team consists of friendly experts from Hill 
 Country MHDD Centers who are dedicated to connecting individuals to 
 available resources in their community. This assistance is provided by 
 grant funding through SAMHSA to help address social determinants of health. 
 Social determinants of health are the aspects of living that affect 
 people’s health\, well-being\, and quality of life\, like access to 
 transportation\, food\, childcare\, education services\, mental and 
 physical health providers\, social connections\, etc. Care navigation is 
 provided through one-on-one meetings on a walk-in basis. We understand that 
 everyone's needs are unique\, and we will determine\, collaboratively\, 
 what barriers and needs exist and work together to develop a personalized 
 plan. Walk-ins only\, no appointment needed.
